Managing diagnoses

We partnered with AAHA to provide our customers with AAHA's diagnostic terms, which include over 10,000 diagnoses.  In addition to these though, we provide the option to create custom diagnoses.

Assigning Diagnoses
There are several ways to assign a diagnoses to a patient, including from the patient's history, from the patient snapshot, and in a medical note that contains the diagnosis widget. 

To assign a diagnosis from the patient's history:
  1. Navigate to the patient's profile
  2. Click on "history" and select "new diagnosis"
  3. Complete the form that appears
To  assign a diagnosis from the patient's snapshot:
  1. Navigate to the patient's profile
  2. Click on "history" and select "view snapshot"
  3. Click on the "plus" icon next to the "open diagnoses" section
  4. Complete the form that appears 
To assign a diagnosis from a medical note:
  1. Navigate to the "assessment" section of the medical note
  2. Click on the "plus" icon next to the "diagnoses" section
  3. Complete the form that appears
The diagnosis widget is included by default in the Basic, Simple, and Standard SOAP templates.  Custom medical notes need to include the widget in order for it to appear.  Learn more about working with custom medical note.

Managing Custom Diagnoses
If the diagnosis you wish to select is not included in AAHA's diagnostic terms, you'll have the option to add a custom diagnosis for use in your practice.  Custom diagnoses are managed under the "custom diagnoses" tab in the Patient Dashboard, but can be added anywhere a new diagnosis can be added by selecting the "Add New" option.  

To add a custom diagnosis from the Patient Dashboard:
  1. Click on "Patients" at the top of the page
  2. Click on "custom diagnoses" and select the "new diagnosis" option
  3. Complete the form that appears
Prior to a custom diagnosis being used, it can be deleted from the Patient Dashboard as follows:
  1. Click on "custom diagnoses" and select the "view active diagnoses" option
  2. Click on the gear icon for the diagnosis you wish to delete, and select the "delete diagnosis" option
  3. Confirm your intent to delete the diagnoses
After a custom diagnosis has been used, it can no longer be deleted but it can be deactivated so that it will no longer be available for selecting when a new diagnosis is assigned to a patient.  To deactivate a custom diagnosis:
  1. Click on "custom diagnoses" and select the "view active diagnoses" option
  2. Click on the gear icon for the diagnosis you wish to deactivate, and select the "deactivate diagnosis" option
  3. Confirm your intent to deactivate the diagnoses
